Lip Lift Surgery

A lip lift shortens the distance between the upper lip and nose, helping the upper teeth show naturally again when you talk or smile. It creates a softly lifted, naturally fuller‑looking lip with better definition. The lip lift procedure is often chosen to reverse age‑related lip elongation and provide a long‑lasting alternative to fillers, without adding volume to the lips.

  • A longer distance between the nose and upper lip (known as a lengthy philtrum)
  • Reduced definition or volume of the upper lip
  • Mild sagging or drooping of the skin beneath the nose
  • A smile where the upper teeth show naturally again, creating a more balanced, defined, and expressive look.

The surgical procedure aims to:

  • Shorten the distance between the upper lip and nose

  • Lift and refine the shape of the upper lip

  • Reveal more of the upper teeth when smiling or talking

  • Reduce fine lines above the lip area

  • Create a natural, balanced look that complements your facial features

Overall benefits:

  • Provides a long-lasting surgical option for upper lip definition

  • Shortens the space between the nose and upper lip to address facial balance

  • Reveals more of the upper teeth when smiling, improving smile appearance

  • Creates a fuller, more defined upper lip shape without added volume from fillers

Dr ILLIE encourages thorough consultations to:

  • Ensure understanding of the procedure, recovery, and outcomes
  • Help make an informed decision
Am I a good candidate?

Lip lift may be suitable for individuals who:

  • Presence of concerns related to the upper lip area, such as a longer distance between the nose and upper lip, reduced lip definition, or mild sagging of the skin beneath the nose
  • Good overall physical health, with no medical conditions that could affect healing or increase surgical risks
  • Non-smoker status, or willingness to stop smoking before and after surgery to support optimal recovery
  • Stable psychological health, with a clear understanding of the procedure, recovery process, and realistic expectations about the results
  • Commitment to following pre- and post-operative care instructions provided by Dr ILLIE and his team
  • Realistic outlook on the outcomes, recognising that the goal is a natural and balanced enhancement tailored to your individual features
    Procedure Options

There are different approaches to lip lift, and the best option will depend on your individual needs and goals. Dr ILLIE will recommend the most suitable technique after a thorough assessment. The final choice of technique will always be based on clinical assessment and your individual preferences.

    Subnasal Bullhorn Lip Lift

    • Involves a bullhorn-shaped incision hidden beneath the base of the nose, within the natural crease
    • Removes a small strip of skin to reduce the distance between the nose and upper lip (philtrum)
    • Elevates the central and lateral parts of the upper lip for a balanced adjustment
    • Suitable for individuals with a longer upper lip in proportion to other facial features
    • Helps improve lip position without adding volume
    • Requires good skin elasticity for optimal healing and natural results
    • Some visible scarring may occur but typically fades and remains discreet
    • Not suitable for all nose and lip shapes; assessment by Dr ILLIE is essential

    Corner Lip Lift (Smile Lift)

    • Involves small incisions at the corners of the mouth
    • Lifts downturned edges of the lips to create a more uplifted and approachable expression
    • Often combined with other lip lift techniques for a comprehensive result
    • Suitable for those noticing a downward turn at the mouth corners
    • Provides a subtle change that improves overall mouth shape
    • Minimal scarring due to small incision size and careful placement
    • Requires commitment to post-operative care for best outcomes

    Lip Lift Surgery

Surgical procedure duration: Approximately 1-2 hours.

Hospital stay: Usually performed in our procedure rooms.

Anaesthesia Information: Lip lift surgery is typically performed using local anesthesia.

    Post-operation recovery:

    • Most patients return to work within 5 to 10 days
    • Avoid heavy lifting and intense exercise for approximately 4 weeks
    • Driving is generally not recommended for 1 to 2 weeks following surgery
    • Smoking: It is important to stop smoking at least 6 weeks before and after the procedure to support optimal healing

    Your surgical team includes:

    • Dr Vlad ILLIE, a specialist plastic surgeon with international training across leading Australian and global centres.
    • Tania, our highly experienced nurse, who assists throughout your surgical journey, supports Dr ILLIE during the procedure, and helps ensure your comfort, safety, and smooth recovery.

    Expected outcomes:

    • A shortened distance between the nose and upper lip, creating a more balanced facial profile
    • A subtly fuller and more defined upper lip appearance
    • Natural-looking enhancement tailored to your unique facial features
    • Improved harmony with other facial features, especially when combined with procedures such as facelift or eyelid surgery

    Surgical steps:

    • Making a precise incision according to the chosen lip lift technique (e.g., subnasal bullhorn, corner lift, a combination of both)
    • Removal of a small strip of skin or tissue to shorten the distance between the nose and upper lip or lift the lip corners
    • Careful lifting and repositioning of the upper lip to achieve a balanced and natural appearance
    • Meticulous suturing of the incision to minimise scarring and support optimal healing
    • Application of dressings and post-operative instructions provided to support recovery
    Surgical Considerations
    Facial Anatomy & Ageing

Over time, natural aging changes the shape and structure of the lips, especially the upper lip. In younger faces, the space between the base of the nose and the top of the lip (called the philtrum) is short, and the upper teeth are more visible when talking or smiling. As we get older, this area naturally lengthens, the lips thin out, and less of the upper teeth are visible.

One major reason for this change is the breakdown of collagen, leading to loss of skin elasticity. With age, the skin around the mouth becomes thinner, while the muscles that shape the lips lose some of their strength. Fat beneath the skin also decreases, and the bones of the midface gently recede inward. Together, these changes cause the upper lip to look longer and flatter, and the once-full vermilion (the pink part of the lip) appears less defined.

The aging process doesn’t just affect the soft tissues — it also involves changes in bone structure. The maxilla (the upper jawbone) shifts slightly backward with age, which reduces support for the lip. At the same time, fat pads in the cheeks descend, making the midface look flatter and further exaggerating the length of the upper lip. This combination can disrupt the balance between the nose, lips, and chin, changing overall facial proportions.

Other visible signs of lip aging include fine vertical lines above the lip, sometimes called “smoker’s lines,” though they appear in non-smokers as well. These lines form due to constant movement of the lip muscles paired with thinner skin that no longer has the same resilience. The Cupid’s bow (the natural “M” shape at the centre of the lip) also loses definition. In addition:

    • Lips appear thinner and less projecting

    • The upper lip covers more of the teeth, reducing youthful dental show

    • Shadows and wrinkles around the mouth become more noticeable

A lip lift procedure is designed to restore balance to the lower face. By removing a small strip of skin just beneath the nose, the surgeon shortens the philtrum and gently raises the upper lip. This change allows the upper teeth to show more when the face is at rest. The procedure also enhances the curvature of the Cupid’s bow, improves lip definition, and restores the proportions of the lips, nose, and chin — results that last longer and look more natural than fillers alone.

Philtrum: The vertical groove between the base of the nose and the upper lip; its length and shape influence lip position and appearance

Cupid’s Bow: The curved shape of the upper lip’s vermilion border, important for lip definition

Vermilion Border: The edge where the lip’s coloured tissue meets the surrounding skin; a key area for direct lip lift incisions

Orbicularis Oris Muscle: The circular muscle around the mouth that controls lip movement and expression

Nasolabial Fold: The crease running from the side of the nose to the corner of the mouth, which can affect lip contour and facial expression

Skin and Subcutaneous Tissue: The layers of skin and underlying tissue that are adjusted during the lip lift to achieve the desired lift and shape

Blood Supply and Nerves: Important vessels and nerves in the upper lip area that are carefully preserved during surgery to maintain sensation and function

      Types of Incisions

The choice of incision depends on the extent of correction needed and the areas targeted:

      Subnasal Bullhorn Lip Lift Incision

      • Incision shaped like a bullhorn placed just beneath the base of the nose
      • Hidden within the natural crease under the nostrils for discreet scarring
      • Removes a strip of skin to shorten the philtrum (distance between nose and upper lip)
      • Elevates the central and side portions of the upper lip simultaneously
      • Suitable for individuals with a longer upper lip or reduced lip projection
      • Allows for a subtle lift without adding volume to the lips
      • Scarring generally fades well due to its concealed location
      • Requires careful planning to maintain natural nasal contours

Corner Lip Lift (Smile Lift) Incision

      • Small incisions made at the outer corners of the mouth
      • Targets downturned mouth corners to create a more uplifted appearance
      • Incisions are discreetly placed to minimise visible scarring
      • Often combined with other lip lift techniques for overall lip enhancement
      • Suitable for individuals with a downward turn of the mouth edges
      • Provides a gentle lift that improves the mouth’s shape and expression
      • Minimal tissue removal, focusing on repositioning lip corners
      • Healing time is typically short with low risk of complications
      Effectiveness
      • Provides a permanent adjustment to the position and shape of the upper lip
      • Can be combined with other facial procedures, such as facelift or eyelid surgery, to support overall facial balance
      • Advances in surgical technique and incision placement have improved the consistency and natural appearance of results

      Lip Lift vs Lip Fillers

      • A lip lift is a surgical procedure that shortens the distance between the nose and the upper lip
        • The result is fuller, naturally shaped lips with long-lasting effects
        • The lack of added volume from the filler makes the lips appear natural
        • Involves a longer recovery period and carries the risks associated with surgery compared to lip fillers
      • Lip fillers are non-surgical injections that add temporary volume and create a more pronounced lip shape
        • Fillers have minimal downtime and effects that can be adjusted or reversed, but require repeat treatments every 6 to 12 months
      • The choice depends on your personal goals and preferences regarding permanence and procedure type
      Potential Risks and Complications

As with any surgical procedure, Lip Lift surgery involves certain risks and potential complications. While most patients experience good healing, possible risks include:

      • Anaesthesia Risks: Reactions to anaesthesia, whether local or general, are possible. It is important to discuss your full medical history with your surgeon and anaesthetist beforehand.
      • Infection
      • Bleeding or bruising
      • Visible scarring
      • Temporary or permanent numbness around the surgical site
      • Asymmetry or uneven appearance of the lips
      • Possible need for revision surgery to address any concerns

During your consultation with Dr ILLIE, you will have a conversation about all potential risks and complications related to this procedure. He will also provide you with a document that explains the possible risks and complications in detail.

        • Careful patient selection and preparation, including managing any medical conditions.
        • Strict adherence to infection control and surgical safety checklists.
        • Post-operative monitoring and follow-up appointments to detect and manage any issues early.
        • Providing detailed aftercare instructions to support recovery.
          Recommendations for Patients

Maintain open and honest communication with Dr Vlad ILLIE throughout your surgical journey. It is important to provide a complete and accurate medical history, including any medications, allergies, or previous procedures. Discuss any concerns or questions you have, as well as your expectations, to ensure a clear understanding of the procedure and realistic outcomes.

Follow all preoperative instructions carefully. This may include:

          • Adjusting or temporarily stopping certain medications as advised to reduce risks during surgery.
          • Ceasing smoking at least six weeks before the procedure to support better healing and reduce complications.
          • Maintaining a balanced diet and staying well-hydrated to prepare your body for surgery.
          • Arranging for support on the day of surgery and during the initial recovery period, as you may have limited mobility.

Adhere strictly to postoperative care guidelines provided by Dr ILLIE and his team. This includes:

          • Proper wound care and hygiene to prevent infection.
          • Avoiding heavy lifting, strenuous exercise, and smoking for the recommended duration to promote healing.
          • Taking prescribed medications as directed to manage pain and reduce inflammation.
          • Attending all scheduled follow-up appointments to monitor your recovery progress.

Be vigilant for any signs of complications and contact the clinic promptly if you experience:

          • Severe or persistent pain that is not relieved by medication.
          • Redness, warmth, swelling, pus, or fever, which may indicate infection.
          • Excessive bleeding or swelling beyond what was explained during your consultation.
          • Any other unexpected symptoms or concerns regarding your recovery.

Following these recommendations carefully helps to minimise risks, supports effective healing, and contributes to achieving the best possible results from your procedure.

          Combined Surgical Interventions

Dr Vlad ILLIE often performs combined surgical procedures to address multiple areas in a single operation, providing a comprehensive approach to facial rejuvenation and functional improvement. Combining procedures can offer the benefit of a single recovery period and a more harmonious overall result.

Lip Lift Surgery + Facelift (Rhytidectomy)

These two procedures are often performed together. Facelift surgery addresses sagging skin and soft tissues in the lower face and neck, complementing the lip lift by improving overall facial contours.

Lip Lift Surgery + Eyelid Surgery (Blepharoplasty)

The lip and eye areas are usually the first to present with wrinkles. The procedure improves the appearance of the upper and/or lower eyelids by removing excess skin and fat.

Combining these procedures allows for a more harmonious and natural restoration of facial balance and ageing signs across multiple areas, with careful planning to ensure patient safety and optimal recovery.

how Dr Vlad ILLIE’s Approach Stands Out

Dr Vlad ILLIE is a highly qualified Specialist Plastic and Reconstructive Surgeon based in Sydney, with extensive experience in facial aesthetic and reconstructive surgery.

After finishing his plastic surgery training in Sydney, Dr ILLIE also completed two international fellowships in Oxford, UK and Stuttgart, Germany, specialising in microsurgical reconstruction, complex facial and breast revision surgery, and advanced cosmetic techniques. His background includes managing challenging reconstructive cases, including head and neck reconstruction, which contributes to his precise surgical skill set.

                His results distinguish by:

                1. Minimal Visible Scarring
Incisions are carefully placed in natural creases beneath the nose or at the corners of the mouth, ensuring scars are discreet and tend to fade over time, in line with best Australian surgical standards.
                2. Preservation of Natural Lip Movement
Surgical techniques prioritise maintaining normal lip function and expression, avoiding stiffness or unnatural tightness while improving lip position and shape.
                3. Balanced Adjustment of Lip Proportions
Attention is given to the overall facial structure to achieve a balanced appearance between the upper lip, lower lip, and surrounding features without overcorrection.
                4. Natural Lip Contour without Excessive Tightening
The procedure avoids removing too much skin or tissue, preventing a “pulled” or artificial look, and focuses on preserving the lip’s natural anatomy.
                5. Maintenance of Lip Volume and Softness
Where appropriate, conservative tissue handling is used to maintain the natural fullness and softness of the lips, avoiding a hollow or overly thin appearance.
                6. Use of Advanced Surgical Techniques
Deeper tissue layers are addressed during surgery where necessary, to provide longer-lasting and natural results, rather than relying solely on skin removal.
                7. Customised Surgery Based on Individual Anatomy and Ageing
Each procedure is tailored to the patient’s unique facial features, skin quality, and ageing pattern, avoiding a “one-size-fits-all” approach.
                8. Comprehensive Pre- and Post-Operative Care
Patients receive thorough education, clear post-operative instructions, and ongoing support to encourage optimal healing and address any concerns during recovery.
